Course Change Request Form

Please use this webform to indicate a change on your curriculum form on file in the STS Office. All changes must be submitted in or before Week 2 of the applicable quarter for approval before the add/drop deadline. An STS advisor will be in touch with you.

NOTE: Only fill out fields applicable to your change requests - you may not need to fill out the entire webform.


Option 1 - Request Course Change (replace/remove/add/swap)

You will need a copy of your approved curriculum form in order to indicate which course you would like to replace. If you are changing 3+ courses per quarter, it is recommended you come into the STS office to update your form - please email Sierra ( for an appointment.

Option 2 - Update Course Quarters

Are you keeping the same course but changing the quarter you originally planned to take it? Please indicate the new intended quarter below & your original form will be updated by an STS advisor.
